I, the undersigned, do hereby consent and agree that Canadian Pole Sports and Arts Federation, its employees, or agents have the right to take photographs, videotape, or digital recordings of me beginning on July 5, 2017, and ending on July 5, 2017 and to use these in any and all media, now or hereafter known, and exclusively for the purpose of Promoting the Canadian Pole Sports and Arts Federation and its Affiliates. I further consent that my name and identity may be revealed therein or by descriptive text or commentary.
I do hereby release to Canadian Pole Sports and Arts Federation, its agents, and employees all rights to exhibit this work in print and electronic form publicly or privately and to market and sell copies. I waive any rights, claims, or interest I may have to control the use of my identity or likeness in whatever media used.
I understand that there will be no financial or other remuneration for recording me, either for initial or subsequent transmission or playback.
I also understand that Canadian Pole Sports and Arts Federation is not responsible for any expense or liability incurred as a result of my participation in this recording, including medical expenses due to any sickness or injury incurred as a result.
I represent that I am at least 18 years of age, have read and understand the foregoing statement, and am competent to execute this agreement.
Hold HARMLESS:
Sport/Athletic Event: Aerial Arts Training (pole sports and arts, lyra, silks, hammock, and acrobatics)
Date of Event: Entirety of 2017/2018
1. In consideration for registering to participate in a competition for the above named sport for and on behalf of myself and Canadian Pole Sports and Arts Federation, I hereby Release, waive, discharge, and covenant and agree not to sue, and agree to hold harmless and not hold Canadian Pole Sports and Arts Federation, its coaches, volunteers, faculty, staff, executives, officers, trustees or 3rd party vendors or others involved in conducting the try out for this sport harmless and free from an responsibility or liability. I agree to release all of the above named employees or servants of Canadian Pole Sports and Arts Federation from any and all claims and demands or injury, including death or broken bones, head or other physical or emotional injuries I may sustain by participating in the activities produced by the CPSAF.
2. I understand that different sports may present risks to me and/or my property, including concussions, broken or sprained ankles, head and other bodily injuries, as well as emotional or psychological injuries. I understand that I cannot control the conduct of other parties and participants involved in the try outs and there will be times in the try outs that I will be in hard physical contact with other people and/or equipment that may cause me injury.
3. I am fully aware that there are inherent risks involved in this activity, and I choose to voluntarily participate in said activity with full knowledge that said risks of loss, property damage or personal injury, including death, that may be sustained by me. I further agree to indemnify and hold harmless the RELEASEES form any loss, liability, damage or costs, including court costs and attorney fees that may occur as a result of my participation in this activity.
4. By selecting "Yes I agree" on this Agreement Not to Sue and Agreement to Hold Canadian Pole Sports and Arts Federation and its representatives harmless, I acknowledge and represent that I have read the foregoing Agreement and understand it and sign it voluntarily as my own free act and deed,. There have been no oral representations, statements or inducements apart from the agreement that has been reduced to writing made to me. No promises have been made about my potential winning of an award in the Competition for which I am participating or my child.
I understand all of the above applies to the child or children to which I am the parent or legal guardian
